Chronic disease self-management (ES-8034-11)
In this unit, Jann Anderssen discusses the self-management model for managing chronic diseases from the perspective of the patient and the medical practitioner.

Learning Objectives:

  • Understand the issues important to people with chronic disease
  • Be aware of the impact of chronic disease on all aspects of a person's life

This education program is developed and delivered for The University of Queensland, by The University of Queensland (UQ) Health Insitu. UQ Health Insitu is the lifelong learning arm of The University of Queensland Faculty of Health Sciences.

Expert presenter:
Jann Anderssen
Jann Anderssen is a physiotherapist with a background in Community Health in both rural and urban settings. Her experience in these settings led to a growing interest in health promotion and education, particularly in the area of chronic diseases such as arthritis and osteoporosis. In this role Jann ran community based self-management programs for about ten years. Jann has been working at Arthritis Qld since 1998, where she is able to pursue this area of expertise as a Master Trainer for the Chronic Disease Self-management courses. Her interest in and understanding of the philosophy of chronic disease self-management has led to opportunities to present workshops and lectures on this topic to health professionals, undergraduate and postgraduate students.
