Advanced diabetic care: patient assessment forms (ES-8081)
This unit contains four downloadable forms for GPs to assist with the assessment and ongoing management challenges of patients with potential and confirmed type 2 diabetes. They provide a systematic approach to patient note taking and management planning, and indicate the relevant clinical parameters for best practice management, timing of specific therapeutic interventions and/or diagnostic investigations, as well as patient self-management strategies.

Learning Objectives:
- Have access to in-practice tools to assist with implementing a systematic approach to the assessment and continuing management of patients with potential and confirmed type 2 diabetes
- Know the clinical parameters and the frequency required to meet the Annual Cycle of Care management review criteria of patients with type 2 diabetes
- Have a systems approach to improving patient safety for people with type 2 diabetes
