These days, it’s all about teams – particularly for chronic disease management. Who should be on your team? How well is your team functioning? How do practice registers, recall systems and internal staff processes deliver the best care for our chronically ill patients?
Using a team approach to optimise care of people with chronic disease (ES-8032) provides the answers to these questions. You’ll discover the components of the effective clinical team, including: internal (within your surgery) and external (including community professionals), practice HR structuring, use of office procedure and protocols, how to employ performance measures to evaluate team operation, and the leadership requirements for the individual at the helm – be that you or your practice manager.
What sort of processes do you have in place to ensure consistency for your patients? In Practice systems and how to use them to better manage chronic disease (ES-8031), you’ll learn everything you need to know about practice systems: appropriate booking, use of a recall/reminder system to track your chronic disease patients, consultation types, and how to use your entire office staff as part of the team. This course will give you confidence in your ability to both create and maintain a successful practice system.
Using EPC items to optimise chronic disease mgmt (ES-8033) explains the major item numbers for chronic disease management. You’ll learn the proper coding for a team care arrangement (TCA) plan and how to set out your GP Management Plan.
